Guidelines for Scholarship Awards 2009- 2010

 

 

Instructions:

 

Students who meet the eligibility criteria must submit the following documents together in one package postmarked no later than Monday, December 15, 2008.

 

  1. An application form (see reverse) including:
  2. Personal statement, additional information and signature
  3. A teacher/administrator referral form (enclosed – yellow sheet) to be completed and signed by an educator who knows the candidate and can evaluate his or her academic performance.
  4. A copy of the student’s most recent academic transcript or report card. (You may be asked to provide an official transcript or report card at a later date.)

 

*   Winners will be asked to provide a photograph.

 

 

Mail all application materials (items 1-4 stated above) in an envelope postmarked by Monday, December 15, 2008 to:

Eligibility Requirements:

 

To be eligible for consideration, candidates must meet all five criteria:

 

  1. Have a specific print disability such as a learning disability, physical disability or visual impairment.
  2. Be registered with RFB&D either individually or through a school, and have used RFB&D’s audio books within the last eighteen months.
  3. Be currently enrolled as a junior or senior in high school at either a public or private school, or be currently enrolled as a college freshman or sophomore.
  4. You may apply multiple years but are only eligible to win once.
  5. Have an overall grade average of 3.0 or above (B average or above), or have demonstrated considerable improvement in grades.
  6. Be an Arkansas, Louisiana, New Mexico, Oklahoma or Texas resident.

Personal Statement:

Please type your name on your personal statement.  The essay should be no longer than two double-spaced pages. A parent or teacher may help with typing the essay but the content must be the student’s original work. Write an essay in response to one of the following:

 

Describe how you handled your school work before RFB&D. How did you learn about RFB&D? What kind of difference has using RFB&D’s Audiobooks made for you?

How would you explain to students with print disabilities the reasons they should try using audio books?  What should be done to help other students, teachers or family members become more aware of the many intelligent students who cannot effectively learn from reading books, and how audio books help these students achieve success?

2)      List any of your achievements or accomplishments

3)      List extracurricular or community activities in which you are involved
